Participating Insurance Information

Please refer to your insurance card for plan participation. For further assistance, please contact your insurance company or speak to your human resource department.
Our insurance coordinator deals with many different insurance companies that offer many different plans. It is almost impossible to accurately estimate our patients' insurance co-payment. Many insurance companies will not give out fees until after the treatment is completed. Dealing with these companies can be difficult and time consuming. As a courtesy, we will submit a pre-determination to your insurance company allowing you to know your out-of-pocket expense. Your insurance company offers no guarantee of payment. You are responsible for all treatment not paid by your insurance company. Your prompt remittance is appreciated. We ask that you keep us informed of any changes to your insurance. It is important that all information about you and your insurance is current. If you have any questions, please ask to speak to our Insurance Coordinator. Please be prepared to give the insurance representative's name and the date you called for rapid results.
Co-Payment
All co-payments are payable the day services are rendered. A payment plan is available if needed. Please inform our office prior to scheduling.
